How about a general submit button at the bottom of the custom tool page to generate any and all changes made? There can still be the submit buttons for every marking for minute changes, but for mass editing there will be a button that can save all your work.

It's a little rough and will need some fine-tuning to support everyone's wishes, but I think it'll be better, especially when trying out several new combinations of markings.
